This Call For Service form is a standard form. I can add any options you think are missing. The form displays different options depending on who is selected in the first step. There are “other” options so the bay staff can post anything that is not listed. Feel free to run thru it multiple times. Test it here.
The form submissions are pushed to your match’s dedicated leadership channel on a Microsoft Teams platform. You can double tap a message to acknowledge it so the rest of your team knows you saw it. The channel can also be used for chat across your match leadership before, during and after the match if needed. I am not able to provide a test drive of the chat channel, you can see some screen shots of it here.
